EFA offers several health insurance plans at group rates. Please note, though, that we are not able to provide coverage in all areas.

The plans we offer are available to members only and are handled for us by third-party administrators. The administrators are very efficient and new members are generally enrolled quickly. EFA does not make any money from your participation in our health insurance plans. To remain eligible for any EFA health plan, you must maintain your membership.

HIP (Health Insurance Plan of Greater New York)

HIP bills EFA members directly for their premiums on a monthly basis. Plan enrollment is administered by RBA Insurance Strategies, LLC. HIP plans are available to members of EFA who are self-employed. Members must submit two of the following documents along with the enrollment materials as proof of self-employment:

• Form 1040 (both pages) plus one of the following
  • Current Schedule C (both pages)
  • Schedule K-1
  • Schedule 1120-S
  • Quarterly Wage and Withholding Form NYS-45

Insurance and Coverage (Plan A)

HIP offers one of their most comprehensive plans to members of the EFA. This plan covers in full a broad range of health care and services with no deductible or coinsurance. These include medical services, hospital care, emergency care, skilled nursing facilities, home care, chiropractic e health care, durable medical equipment and prosthetic devices, and inpatient private duty nursing. It also includes prescription drug coverage with a copay of $5/generic and $10/brand name, and mental health care with a $25 co-payment per visit. Monthly rates are effective 4/1/2006.

• Husband/Wife: $ 954.13

Insurance and Coverage (Plan B)

Plan B is an alternative comprehensive plan, covering most of the services in Plan A. Deductibles and co-payments are introduced in Plan B so that we can offer you a plan with a lower monthly premium. For more details, see Plan B.

If you decide to join either of these plans, please be sure to complete the paperwork and return it to RBA with a check payable to RBA Associates for the appropriate amount. Please allow ample time for your application to be processed and to receive your HIP ID card.

These are group plans, available only to self-employed members of the EFA. Continuous membership is required.

The Entertainment Industry Group Insurance Trust (TEIGIT)

Since 1965, The Entertainment Industry Group Insurance Trust (TEIGIT) has administered health insurance plans for members of participating associations. On July 28, 2006, TEIGIT announced the expansion of its plans to include Long-Term Care, Life, Disability, and International Travel Health Insurance. The TEIGIT website has more information on most of these new offerings; if you do not find what you're seeking there, contact TEIGIT directly.

TEIGIT offers health insurance to EFA members in New York State through Oxford Health Plans and GHI; and in New Jersey, Connecticut, Florida, California, and in the Greater Chicago area through CIGNA. TEIGIT also offers individual insurance to members in Texas, Arizona, and Virginia.

CIGNA HMO Dental is available in most parts of the country.

TEIGIT also offers long-term care insurance, international health and travel insurance, and life insurance and disabiity.

Careington International

Careington is not an insurance plan but rather offers EFA members customized discount plans and a premium discount of 10 percent. Careington will waive one-time enrollment fees for EFA member-subscribers. These plans and discounted rates are available nationwide exclusively to EFA member-subscribers.

If a member is not satisfied within 45 days of enrollment, Careington will refund 100 percent of the membership fee (but not the $20 enrollment fee for the Total-Care plan).

Total-Care

Premium for an individual is $26.95/month and for a family (household) is $242/annually. There is a $20 enrollment fee for this plan.

Members interested in these discount plans can go to Careington's websites (www.careington.com or www.cmccare.com) for product details, provider search, and fee schedule of benefits according to state of residence. Please note: Careington's customized rates to EFA member-subscribers are applicable only if application forms are obtained from EFA and payments are mailed by USPS to Marvin Migdol at Careington at address below. Member services will be provided by Careington directly to EFA member-subscribers.

A member may call a physician in advance to determine if he or she is affiliated with a hospital in the Careington plan or may call Member Services at (800) 290-0523 to nominate a physician to the Careington plan(s).

Northeast Dental Plan

Somewhat analogous to HMO plans. Clients choose from a list of affiliated dentists; the area covered is limited.
